Athlete Registration & Required Forms

APS now uses DragonFly for sports and activities, and it makes everything more organized, with easy to use digital forms, health records and team communication tools.


  1. Visit DragonFly and click the 'Log In/Sign Up' button.
  2. Click 'Sign Up for Free' then follow the prompts to create your Parent account with your own email address.
    • Note: Please do NOT create an account with your child’s name or contact information — you will get the chance to add your child soon!
  3. Verify your account with the verification ID sent to your email address.
  4. Click 'Connect to your school' to select 'Parent' as your role and search for your child's school.
  5. After selecting your child's school, click 'Join' to request access. An administrator at your school will approve your request. You do not need to be approved to complete forms for your child.
  6. Click 'Set up your children' and follow the prompts to add your kid(s) and fill out their participation forms. Add any child attending an APS Middle or High School.
    • Note: The only document or page that needs to be uploaded is the "Physical Examination Form" Doctors signature or stamp page. You do not need to upload the entire document.
  7. Complete all required forms, questions and videos listed.
    • If your Cibola student-athlete has not handed in their physical form to the Cibola Athletic Trainer, please upload their physical form onto ‘DragonFly’ after it has been filled out by a parent, student and a health care professional.. 
    • Complete the following forms that will appear in your DragonFly account:
      • Code of Conduct
      • Consent to Treat
      • Life of an Athlete
      • Concussion for Students
      • Consent to Participate
    • Do not upload your student-athlete’s birth certificate.
  8. Watch a few short video tutorials if needed.
