                        Submit form to Coach Martinez email ASAP 

     *All New Athletes who will be playing soccer will need a physical                 prior to training.

Monday, August 10-Wednesday August 12, 5pm-7pm (tentative)

Requirements prior to tryouts

      *Concussion for Students Class Completion/Certificate

      *Life of an Athlete Course Completion/Certificate

      *Athletic Activity Code of Conduct

      *Mandatory Athlete Registration(bring print out of completion)
